COMMUNITY HEALTH REPRESENTATIVE

Monday to Friday – 7 hours/day
Wabasca, Alberta
Bigstone Health Commission is seeking an experienced Community Health Representative to join our talented Health Team. This is a rewarding opportunity if you have an appreciation for First Nation culture, a desire for continuing professional development and enjoy the support and collaboration from our multidisciplinary team and various team and various other professionals and agencies.
Duties and Responsibilities:
• Conducts primary, secondary and tertiary preventive services, provide care and support to clients, families and the community.
• Conducts home visits to provide accurate health information and education, counseling and support to individuals and families
• Promote awareness of various community health services programs
• Provide preventative public health screening program under the direction of the Nurse in Charge
• Plans, coordinates, educates and contacts client’s regarding community health program services
• Community advocacy: works in collaboration with other health team members, develop partnerships with community based agencies and/or external health care providers
• Cultural liaison
• Administration and program requirements
Knowledge, Skills, and Abilities:
• Must have a high degree of professionalism.
• Possess excellent interpersonal skills. communication and organizational skills.
• Must be dependable and reliable
• Confidentiality is strictly enforced.
• Must be proficient with computers.
• Ability to speak and understand the Cree language and culture is an asset
• Excellent communication and interpersonal skills
• Must be willing to travel
Education and Experience:
• Community Health Representative (CHR) Certificate
• Grade 12 Diploma
• Minimum 2 years work experience in this field
• Must have or obtain within 6 months of hire, Standard First Aid/CPR certification, WHMIS and TDG
• Valid Class 5 driver’s license and reliable transportation
